Why is IQVIA up ~12% today — the S&P 500's top gainer?

IQVIA (NYSE: IQV) was the S&P 500's top gainer on Tuesday, ripping about 12% to roughly $240 — StockStory clocked the morning pop at 12.9%, Zacks at 12.35%. That's a rare double-digit day for a company worth north of $40 billion, and it happened while the AI-chip trade was getting torched all around it.
What lit the fuse
Q2 numbers — and one figure in particular:
- Revenue $4.37B, up 8.7% YoY, past the ~$4.29B street estimate.
- Adjusted EPS $3.15, up 12.1% YoY, ahead of the ~$3.02 consensus.
- The real mover: R&D Solutions booked a record $3.15B of net-new work, up 19% YoY — a 1.22x book-to-bill. In plain English, IQVIA signed $1.22 of fresh clinical-trial work for every $1 it burned through.
- Then management raised full-year guidance — revenue to $17.28–17.48B (from $17.15–17.35B) and adjusted EPS to $12.80–13.00.
Why the market cared this much
For two years the bear case on the CROs — the contract-research firms that actually run drug trials — was that biotech funding had dried up and Big Pharma was slashing R&D, so trial demand would fall off a cliff. A record order book with book-to-bill back above 1.2x is the cleanest evidence yet that pharma's still spending on drug development, not cutting. That's why IQV gapped straight toward its 52-week high near $244 (set in January).
The ripple
IQVIA is the biggest CRO, so its bookings get read as an industry tell. The knock-on lands on peers who live off the same pharma trial budgets — ICON (ICLR), Fortrea (FTRE) and Charles River (CRL); smaller rival Medpace (MEDP) already popped ~17% on its own record book last week. It also fit the day's bigger tape: money bailing out of the crashing AI-memory names (SanDisk −16%, Korea's KOSPI −11%) and into cash-generative, defensive healthcare. IQVIA sat right in the middle of that rotation.
The one risk that flips it
It's all about bookings cadence. Watch next quarter's R&D Solutions book-to-bill: if it slips back under ~1.2x, the "pharma is still spending" thesis unravels and today's re-rating hands some back. On the chart, the level is the 52-week high near $244.29 — a clean break confirms the breakout; a rejection there caps it.
